For case managers, discharge planners and community partners

Finally, a housing option built for the gap you're working in.

Your client is too independent for a care setting and can't get through an apartment's approval process. Somewhere offers affordable, furnished shared living for independent adults across the Phoenix metro, with individualized screening and straight answers. Everything you need to make a referral is on this page.

  1. Send the referralUse the form below, call or email. Basics only, no medical records.
  2. Initial screeningWe follow up to understand housing needs, budget, timeline and eligibility.
  3. ApplicationThe applicant completes the full application and income and background screening.
  4. PlacementWe match the applicant to the right house and room. Tours in person or by video.

Refer Someone

Send us the basics. We'll take it from there.

We don't need their life story or their medical chart to say hello. Tell us who you are, how to reach you and the timeline, and we'll follow up to talk it through.

Please don't submit medical records, diagnoses, Social Security numbers or banking information. This form starts a housing conversation and isn't a clinical referral portal or for emergencies.
